Tennessee residents may be eligible to receive a free CapTel telephone through the Tennessee Telecommunications Devices Access Program (TDAP), which loans telephone equipment to people with hearing loss who qualify financially and medically.

To apply, download the application and instructions below (you will need Adobe Acrobat Reader to view these files). Then, follow the directions to complete the form and submit it.
