Apply for a CapTel Phone

Minnesota residents who have a hearing loss, speech, or physical disability that limits their use of a standard phone, and who meet specific income eligibility guidelines, may qualify to receive a free CapTel phone through the Minnesota Telephone Equipment Distribution (TED) program. The program offers both the CapTel 200 and the CapTel 800i. For more information on the TED Program, please visit their Web site at: www.tedprogram.org.

Qualified residents can download an application. Then, follow the instructions to complete and submit the form*.
